Terms of Service

These Terms of Service ("Terms") govern your use of the website located at btl-injections.com (the "Site") and the aesthetic and wellness services offered by Between the Lines Injections ("we," "us," or "our") at 4411 S 2950 E, Holladay, UT 84124 (the "Practice"). By booking an appointment, using the Site, or receiving services, you agree to these Terms. If you do not agree, please do not use the Site or our services.

1. Eligibility

• You must be at least 18 years old to book or receive services, unless accompanied by a parent or legal guardian who provides written consent where permitted by law and by our clinical policies.
• You confirm that the health information you provide during intake and consultation is accurate and complete, and you agree to promptly update us of any changes (e.g., new medications, pregnancy).

2. Our Services

We offer aesthetic and wellness services which may include

neuromodulators (e.g., Botox, Dysport, Jeuveau, Letybo), dermal fillers, and other injectable or energy-based treatments, performed by Krista Corbett, APRN, NP-C, a licensed Advanced Practice Registered Nurse practicing independently under full practice authority granted by the Utah Nurse Practice Act. Descriptions of services on our Site are for general informational purposes and do not guarantee specific results.

3. Consultations and Informed Consent

• Before any injectable or aesthetic procedure, you will complete a health history intake and a consultation to assess candidacy.
• You will be asked to review and sign a separate, procedure-specific informed consent form describing the risks, benefits, alternatives, and expected recovery for that treatment before it is performed. These Terms do not replace that consent form.
• Results vary by individual and are not guaranteed. Aesthetic treatments carry inherent risks, including but not limited to bruising, swelling, asymmetry, allergic reaction, and, rarely, more serious complications, which will be discussed with you prior to treatment.
• You agree to disclose all relevant medical history, including pregnancy, breastfeeding, allergies, medications, and prior cosmetic procedures. Failure to disclose accurate information may affect your safety and candidacy.

4. Scheduling, Cancellations, and No-Shows

• Appointments may require a credit card on file or a deposit to reserve your time.

Cancellations or reschedules made less than 24 hours before your appointment, and no-shows, may be subject to a fee ranging from $50 to $850 depending on the service reserved, charged to the card on file or deducted from your deposit. The applicable fee for your treatment will be disclosed to you at the time of booking.

• We reserve the right to reschedule or decline appointments due to provider availability, clinical judgment about candidacy, or safety concerns.

5. Payment, Pricing, and Refunds

• Prices are as listed at the time of your appointment and are subject to change without notice for future bookings.
• Payment is due at the time services are rendered unless other arrangements (e.g., financing, packages) are agreed to in writing.

• Touch-up appointments, if offered, are subject to our separate touch-up policy and time window, communicated to you at the time of treatment.

6. Photo/Media Release

We may take before-and-after photos and video for your medical record. We will only use your photos, video, or testimonials for marketing, social media, or educational purposes if you sign a separate, optional media release. 7. Medical Disclaimer

Information provided on our Site, in consultations, or in marketing materials is for general informational purposes only and is not a substitute for individualized medical advice. Always disclose your full medical history to your provider and follow your provider's pre- and post-treatment instructions.

8. Website Use and Intellectual Property

• All content on the Site --- including text, images, logos, and before-and-after photos we own or have rights to --- is our property or that of our licensors and may not be copied or used without permission.
• You agree not to misuse the Site, attempt unauthorized access to our systems, or submit false information through our booking or contact forms.

9. Limitation of Liability

To the fullest extent permitted by Utah law, Between the Lines Injections and its owners, providers, and staff will not be liable for indirect, incidental, or consequential damages arising from your use of the Site or services. Nothing in these Terms is intended to limit liability for gross negligence, willful misconduct, or claims that cannot be limited or waived under applicable law. Specific risk acknowledgments and liability terms related to your medical treatment are addressed in your signed procedure-specific informed consent form.

10. Indemnification

You agree to indemnify and hold harmless Between the Lines Injections from claims arising out of your breach of these Terms or your provision of inaccurate health information.

11. Governing Law and Disputes

These Terms are governed by the laws of the State of Utah, without regard to conflict-of-law principles. Any dispute arising from these Terms or our services will be resolved in the state or federal courts located in Salt Lake County, Utah [or: through binding arbitration in Salt Lake City, Utah, per (arbitration provider) rules --- choose one approach with counsel].

12. Mobile Messaging (SMS) Terms

By providing your mobile number and opting in to text messaging, you agree to the following terms. These SMS terms apply specifically to our text messaging program.

Types of messages. Messages related to your account, orders, services, appointment reminders, marketing and promotional messages may be received. Frequency may vary.

How to opt out. You can cancel the SMS service at any time by texting “STOP.” After you send “STOP,” we will confirm your unsubscribe via SMS. After this, you will no longer receive messages from us. If you would like to opt back in, you may sign up again as you did previously.

Help. If you are experiencing issues with the messaging program, reply with the keyword “HELP” for assistance, or contact us directly at hello@btl-injections.com or 385-313-8707.

Carrier liability. Carriers are not liable for delayed or undelivered messages.

Message and data rates. Message and data rates may apply for any messages sent to you from us and from you to us. Message frequency may vary. For questions about your text or data plan, please contact your wireless provider.

Privacy. If you have questions regarding privacy, please review our Privacy Policy.

Please do not send medical information by text. SMS is not a secure channel and is not appropriate for protected health information. Text messaging with our practice should be limited to scheduling and general administrative matters.

13. Changes to These Terms

We may update these Terms from time to time. Continued use of the Site or booking of services after changes are posted constitutes acceptance of the updated Terms.
